A unique type of rapidly progressive interstitial fibrosis of the kidney, the Chinese Herb Nephropathy (CHN) has been reported in Belgian woman after intake of Chinese herbs. CHN contains several characteristics that include variable clinical features from progressive renal failure to Fanconi's syndrome, histologic finding of extensive interstitial fibrosis, high risk of urothelial malignancy. We are reporting a case of33 year-old female patient who was diagnosed as CHN. The patient visited our hospital with symptoms of nausea. Laboratory investigation showed metabolic acidosis with normal anion gap, hypokalemia, hypophosphatemia, proteinuria, glycosuria consistent with Fanconi's syndrome. She took Chinese herbs for slimming 4 months before. Renal biopsy showed the features of tubular cell injury with interstitial edema, accompanied by interstitial fibrosis on following biopsy, compatible with Chinese Herb Nephropathy. After 5 months, her renal function was progressed rapidly, therefore peritoneal dialysis was started. This case shows that we should consider Chinese herb as a cause of renal failure. To examine the management status of patients with rheumatoid arthritis (RA) taking glucocorticoids treated at a tertiary hospital rheumatology Practice. A retrospective chart review was performed on 114 RA patients who were noted to be taking glucocorticoids during the period of 2000.7 to 2003.7, and who had been followed up for at least one year period. Use of bone densitometry and use of prescription medication for osteoporosis management were assessed. Thirty one % of patients underwent bone densitometry, and 34% of patients received prescription medication for osteoporosis management. Sixty-six percent of patents who underwent bone densitometry showed osteoporosis with T score of less than 2.5, Osteoporosis medication was prescribed most often in the prednisolone exposure group of at least 7.5mg/d for at least 6 months. Factors associated with not receiving management for osteoporosis included male sex, young age, and premenopausal woman. The use of bone densitometry and prescription medication for osteoporosis were suboptimal. Interventions to improve detection and prevention of glucocorticoid induced osteoporosis are necessary.

We studied the open clinical trial to evaluate the efficacy and safety of TAGOCIN®, teicoplanin, glycopeptide anti microbial agent, which was manufactured in one Korean pharmacy. We selected 15 patients hospitalized with presumed or proven infection due to gram positive organism, judged to require glycopeptiede and administered by intravenous bolus injection at a dose of 200mg TAGOCIN® per day. The efficacy was assessed by bacteriological result, clinical symptom and radiological change. Mean age of 15 patients treated is 66. 13 cases were lower respiratory tract infection, I case was urinary tract infection and I case was soft tissue infection. Gram positive organism was isolated in Seven samples of five patients, and at four patients of them, gram positive organism were eliminated. During treatment, one patients liver enzyme elevated mildly. TAGOCIN® is an effective and tolerated antibiotics against gram positive organism in severe infected patients.
