Immunotherapy has been considered as secondary strategy of IgE mediated allergic disease. It decreases delayed airway inflammation and blocks irreversible airway change but many controversies still exist despite of its remarkable effect. The purpose of this study was to estimate clinical effectiveness of immunotherapy in patients with asthma sensitized to house dust mite and to evaluate related factors. Thirty asthmatic patients visiting allergy clinic of Mock-Dong hospital of Ewha woman university enrolled from March, 1996 to February. All patients were received the 1) Symptom-medication score was significantly improved after treatment (8.9±0.29 ; before IT vs. 3.4±0.20 ; after IT, p<0.05). 2) Methacholine challenge test demonstrated significant improvement (PC20 0.78± 0.09 before IT vs. 1.l8±0.17 after I year of IT, p<0.05). After we divided patients into two groups, group I that showed more than twice increasing in PC20 after I year IT and group II that did not. There was no significant difference between the groups in pre-treatment PC20s (8.75±2.75mg/ml in group I vs. 7.08±1.84mg/ml in group II, p<0.05). 3) Skin prick test demonstrated no significant change (wheel flare ratio in 4) There were no significant correlations for any combination among these three parameters (symptomatic improvement, change of PC20 and disease duration of asthma). 5) In view of side effects, no fatal systemic reaction occurred during treatment period. Only one patient experienced injection-related dyspnea and urticaria that were easily controlled by medication. This study demonstrated that patients with asthma sensitized to house dust mite benefit from specific immunotherapy through symptomatic improvement and reduced nonspecific bronchial hyperreactivity. As for side reactions immunotherapy might be relatively safe if allergen is carefully administered a controlled and supervised environment.

We studied the open clinical trial to evaluate the efficacy and safety of TAGOCIN®, teicoplanin, glycopeptide anti microbial agent, which was manufactured in one Korean pharmacy. We selected 15 patients hospitalized with presumed or proven infection due to gram positive organism, judged to require glycopeptiede and administered by intravenous bolus injection at a dose of 200mg TAGOCIN® per day. The efficacy was assessed by bacteriological result, clinical symptom and radiological change. Mean age of 15 patients treated is 66. 13 cases were lower respiratory tract infection, I case was urinary tract infection and I case was soft tissue infection. Gram positive organism was isolated in Seven samples of five patients, and at four patients of them, gram positive organism were eliminated. During treatment, one patients liver enzyme elevated mildly. TAGOCIN® is an effective and tolerated antibiotics against gram positive organism in severe infected patients.

In order to elucidate the important factors for refractoriness following repeated exercise in exercise-induced asthma(EIA), we investigated the relationship between the change of bronchial hyper-reactivity to methacholine during the refractory period and the degree of refractoriness following the exercise. Ten EIA patients were tested. First visit included methacholine bronchial provo-cation test(MBPT) followed by exercise 1 and repeated MBPT separated by a 60 min. On seco-nd visit, exercise 3 were repeated after 60 min of exercise 2, and refractoriness was measured. The maximal broncial constriction measured by FEV1 was 22.6±4.2% and this reaction was recovered over 95% within 60 min. Methacholine PC20(PC20M) was decreased after exercise in six from 10 EIA patients. There was no significant relationship between the degree of exercise induced bronchial-constriction and the chagne of PC20 after exercise. The bronchial hyper-reactivity to methacholine was in--creased after exercise even with the refracto-riness in some of EIA patients. Refractoriness followed by repeated exercise does not seem to be due to the decreased bronchial hyper-reactivity. Citations Citations to this article as recorded by

We investigated the prevalence of bronchial hyperreactivity and atopic state in patients with chronic cough to see the cause and clinical significance of those factors in chronic cough. We studied 92 individuals who had presented with paroysmal cough over 3 weeks without dyspnea or wheezing. Methacholine bronchial provocation test and allergic skin test with paranasal sinus X ray, symptoms, serum lgE levels and peripheral eosinophils were measured and analyzed. 1) Forty eight percent of patients showed positive hyperreactivity to methacholine provocation, 11 percent had postnasal drip syndrome and 9 percent had both of them, Three precent of patients were thought to have gastroesophageal reflux. However 29% has do specific causes for their cough. 2) Forty four percent of patients were atopic patients and others were non-atopics. There were no significant differences of symptom score, serum lgE levels and peripheral eosinophil count between the two groups. 3) Sixty two percent of atopic patients and 40 percent of non-atopic patient were cough variant asthma with positive reaction to methacholine provocation test. However there was no significant difference of mean PC20 of methacholine between the two group. These results indicated that airway responsiveness and atopic started are important factors for chronic cough. However they were not predictable on the basis of clinical and ordinary laboratory findings.

Buckwheat belongs to the Polygonacea or buckwheat family and is not a true cereral since it does not belong to the Graminiae, or grain family. Its products have long been considered excellent foods for both human and animal consumption. The earlist reference to case of buckwheat sensitivity was a report by Smith in 1909. The authors have experienced one case of buckwheat allergy. The chief complaints were wheezing and dyspnea for several years. The skin tests showed positive reaction to the extracts of buckwheat flour and buckwheat husk. The buckwheat allergy is considered to be the model of the typeIallergy(that is lgE dependent type) proposed by Cooms and Gell. T-Iymphocytes regulate the erpression of a number of surface antigens such as CD4, CD8 and T cell receptor/CD3 complex, during their intrathymir development. The appearance of surface CD3 is heralded by cytoplasmic CD3 expression in T cell precursors. Thus early T cell progenitors may be distinguished by the presence of cytoplasmic CD3 in the absence of surface CD3. In this study, 1 described a technique for the simultaneous detection of intracytoplasmic CD3 and other surface antigens, CD4 and CD8 in the cells permrabilization by NP-40 fo11owing formaldehyde fixation. The technique results in no shift in the light scatter property andno changes in the expression of surface antigens, Tho present technique permitted the detection of cytoplasmic CD3ε and may prove useful in studying the expression of several cytoplasmic and surface antigens during early T cell development.

From April 1981 to March 1985, a clinical study was made at the Intcrnal Medicine department of Ewha Womans University Hospital on 13 patients with IgA nephropathy. The following results were obtained. 1) The mean age of patients was 27 years, the male to femle ratio was 1.2:1. The most common chief complaint was gross hematuria(38%). 2) In urinalysis, hematuria and proteinuria were noted 100% and 92%, respectively. Creatinine clearance was decreased than normal range of creatinine clearance. ASO titer, immunoglobulin and C3 level were normal range. 3) Kidney biopsy finding showed minor mesangeal change in 62% of patients. IgA deposition was noted in all patients.
