Laboratory Diagnosis of COVID-19 in Korea

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the cause of coronavirus disease 2019 (COVID-19), is a type of human coronavirus that causes severe pneumonia, similar to SARS-CoV-1 and Middle East respiratory syndrome coronavirus. In Korea, the SARS-CoV-2 testing has started quickly from February 2020 to respond to the COVID-19 pandemic. In this article, I would like to introduce the characteristics of coronavirus and PCR test methods that play a large role in COVID-19 quarantine measures. Real-time reverse transcription (RT)-PCR is one of the molecular diagnostic method, and it detect SARS-CoV-2 RNA by amplifying SARS-CoV-2 specific RdRp (RNA-dependent RNA polymerase) gene and E (envelope) gene at the same time. Real-time RT-PCR is currently the most reliable test that confirming COVID-19 and is in use worldwide. Real-time RT-PCR test is recommended for COVID-19 confirmatory diagnosis in Korea, but this test requires dedicated equipment, reagents, experienced technicians and laboratory medicine specialists, and it takes about a few hours to a day to report. Rapid molecular testing results in one to two hours with a simple procedure. Antigen test is less sensitive than real-time RT-PCR and can only be used as a secondary role of diagnosis. As the global COVID-19 pandemic progresses, diagnostic testing guidelines and recommendations may vary and will be updated as scientific evidence and experience of the COVID-19 accumulates.

New Molecular Targeted Therapy of Metastatic Colorectal Cancer

Over the past decade, substantial advances have been made in the individualization of therapeutic strategies for metastatic colorectal cancer (mCRC). Treatment strategies have been developed and classified according to their molecular and genetic characteristics based on predictive biomarkers such as microsatellite instability, RAS and BRAF mutations, HER2 amplification, or NTRK fusions. As molecular and genetic predictive tests are routinely performed, new challenges for mCRC treatment strategies are allowed. For patients responding to anti-epithelial growth factor receptor treatments, expanded biomarkers panels enable customized treatment to be selected and the optimal treatment can be determined. Patients with mCRC with the BRAFV600E mutation who did not have effective targeted treatments have effective therapeutic options. Attractive but rare targets, such as HER2 amplification and NTRK fusions, could be a breakthrough and the use of immune checkpoint inhibitors in patients with mismatch repair deficiency/microsatellite instability is the striking revolution. In this review, we summarize the current landscape of targeted therapies for mCRC patients, with a focus on new developments for epithelial growth factor receptor blockade and emerging biomarkers.

Correlation between Systolic Blood Pressure and Triglyceride Level in the Uzbekistan Population

Objectives

: This study aimed to characteristic the systolic blood pressure (SBP), diastolic pressure, pulse pressure, glucose, creatine, and lipid profile. This study also aimed to investigate the prevalence of hypertension and the relationship between hypertension and the lipid profile in Uzbekistan.

Methods

The subjects consisted of 58 Uzbekistan subjects recruited from Ewha Medical Care patients. Blood samples were collected from the patients for the lipid profile and random glucose and creatinine levels. Paired t tests were used for the group means and a chi-square or Fisher’s exact test for categorical variables. A multiple logistic regression analysis was performed.

Results

Among the 58 patients constituting the baseline population, hypertension developed in 42 patients. Among them, the triglyceride (TG) level was significantly higher in the hypertension group than normal group (173.19 vs. 127.06 mg/dL, P=0.014). The SBP had a positive correlation with the TG (r=0.979, P<0.01) and creatinine (r=0.002, P<0.05) levels and also, the pulse pressure had a positive correlation with the cholesterol level (r=0.539, P<0.05). A multivariate analysis (adjusted for age and sex) indicated that there was a positive correlation between the SBP and TG level (r=0.941, P<0.05).

Conclusion

There was a positive correlation between the SBP and TG level in the Uzbekistan population according to this study.

Acute Vulvar Ulcer (Lipschutz Ulcer) in a Sexually Inactive 11-Yearold Girl

Non-sexually acquired genital ulceration, also known as Lipschutz ulcer, is a rare condition that typically occurs in prepubertal and pubertal girls. It can be misdiagnosed as a sexually transmitted disease or even a sign of child abuse, causing great anxiety for patients and their families. It is often accompanied by systemic symptoms such as fever, myalgia, or lymphadenopathy. Several viruses such as Epstein-Barr virus, cytomegalovirus, and mumps virus have been associated with this entity. Furthermore, Mycoplasma pneumonia has rarely been linked to such ulcers in the literature. We present a case of Lipschutz ulcer in a sexually inactive 11-year-old girl.
