The microdiskectomy was developed out of attempts to refine the classical lumbar laminectomy and diskectomy. The ligament-sparing microdiskectomy, a more refined technique, was developed by the author. For this study a series of patients, operated by the present author, were reviewed.
Consecutive patients admitted to Ewha Mokdong hospital between August 1997 and June 2000 who were to undergo virgin, unilateral single-level lumbar ligament-sparing microdiskectomy undergoing unilateral single-level microdiskectomies were included. The technique of the ligament-sparing diskectomy was described in detail.
A total of 83 patients were operated on(M : F/63 : 21), with an average age of 37.2 years. Successful outcome was achieved in 78 patients(93.9%). Surgical complications included one wound infection and one reoperation for residual disk herniation.
The author believe that this technique is safe, can provide acceptable clinical outcome, and has an additional advantage of preserving the epidural fat.
The field of dermatologic surgery has seen trementdous developments in the past few years.
Our purpose was to discuss current statistical data on the dermatologic patients treated by surgical methods at our institute.
We analyzed 2,334 cases of surgeries performed on 1,075 patients at the Department of Dermatology of Ewha Womans University Tongdaemun Hospital during a period of 5 years (1994. 1 - 1998. 12).
The results are summarized as folows:
1) Of the 2,334 cases of surgeries, 846 cases(36.2%) were chemical peelings, 650 cases(27.8%) cryosurgeries, 255 cases(10.9%) CO2, lasers, 209 cases(8.9%) dermabrasions, 113 cases(4.8%) scalpel surgeries, 61 cases(2.6%) Mohs micrographic surgeries, 50 cases(21.1%) sclerotherapy, 48 cases(2.0%) flap repair surgeries, 35 cases(1.5%) punch elevations, 15 cases(0.6%) hair transplantations, 13 cases(0.5%) epidermal grafts, and 2 cases(0.1%) autologous fat/collagen transplantations.
2) The ratio of male to female was 1 : 2.4 and the mean age of patients was 30.3 years.
3) A total of 1,354 cases of diseases were managed surgically : among them, the most frequent disease was scar(39.8%), followed by pigmentary disorders(30.8%), and benign or malignant tumors(21.2%).
4) With respect to the types of scars, acne scar was treated most frequently by chemical peeling (72.7% of 580 cases), atrophic scar by dermabrasion(37.3% of 126 cases), and hypertrophic scar (including burn scars) by cryosurgery(35.3% of 85 cases).
Neural tube defects are a heterogenous group of malformations resulting from failure of neural tube closure during early embryogenesis. Anencephaly is the commonest form of neural tube defect and results from failure of closure of the anterior portion of the neural tube. Anencephaly is characterized by absence of the cranium along with cerebral hemispheres that are rudimentary or absent and risk of recurrence after affected child is 2-3%. Periconceptual folic acid intake may decrease the incidence and recurrence of anencephaly.
Most often, anencephaly is discovered by conventional two-dimensional ultrasonography at the time of attempted biparietal diameter determination for fetal age in the second trimester. Two-dimensional transvaginal ultrasonography has a limitation in a motion of the transducer shaft due to narrow space of the vagina. It is sometimes impossible to obtain information of the whole brain and to miss the fetal CNS(central nervous system) anomalies. Recent advanced three-dimensional ultrasonography has remarkably improved not only surface rendering but also multiplanar analysis of internal structure.
Recently, we encountered one case of recurrent anencephaly that had occurred in a same pregnant woman and three-dimensional transvaginal ultrasonography enabled us to diagnose anencephaly at 113 weeks of gestation. We report this case with brief review of the literatures.
A case report is presented on a patient with Wernicke's encephalopathy secondary to hyperemesis gravidarum. The 30-year-old woman was admitted with a severe dehydrated state, ataxia, dizziness at 17 weeks. Neurologic examination demonstrated nystagmus, gait ataxia, paresthesia. The MR imaging revealed the high signal intensity in both hypothalamus, thalamus, midbrain. The neulolgic signs and MRI findings pointed to diagnosis of Wernicke's encephalopathy. The patient was complicated with hyperthyroidism. The patient was treated with parenteral thiamine and prophylthyiouracil. The pregnancy was terminated by spontaneous abortion at gestationl 18 weeks. A review of the literature published during the last 30 years revealed an additional 25 cases.
