We have investigated in rat liver whether different forms of cytochrome P-450 are alterated in hepatic disorders associated with impaired drug metabolism. Total hepatic cytochrome P-450 is decreased after either bile duct ligation or the administration of estradiol. In contrast, phenobarbital administrated alone increase hepatic content of cytochrome P-450, and when administrated with estradiol the reduction in cytochrome P-450 was prevented. Four forms of microsomal cytochrome P-450 apoproteins, ranging in molecular weight from 50 000 to 58,000, were tentatively identified in a sodium dodecyl sulfate(SDS)-6M urea polyaocrylamide gel electrophoresis system. Phenobrbital administration icreased primarily band IV(50,000 daltons). Bile duct ilgation was associated with a marked reduction in bands II and III while bands II and III were decreased with estradiol benzoate administration. Sumultaneous administration of phenobarbital and estradiol demonstrated a return of band I and an increase in density of bands III and IV. Simultaneous administration of cholic acid and estradiol demonstrated a return of band I and not altered in band III and IV. These studies support the hypothesis that multiple forms of cytochrome P-450 are present in liver microsomal membranes and that alterations in spenific apoproteins may be associated with an increase or a decrease in the functional properties of cytochrome P-450.

Epidemiological Observation of Clonorchis sinensis Infection in Connection with Primary Carcinoma of the Liver

An epidemiological study on the primary carcinoma of the liver was carried out to obtain further information on the causal factor of cholangiocarcinoma and hepatocellular carcinoma during the period of 1997 and 1978. Total 102 hospital records of the primary carcinoma of the liver from 3 Hospitals were collected and analysed. Among 102 primary carcinomas of the liver, cases of hepatocellular carcinoma and cholangiocarcinoma were 96 (M83, F13) and 6 (M6, F1) respectively and the ratio was 16.0 : 1. Total cases of the primary carcinoma of the liver combined with clonorchiasis were 18 (M16, F2). Prevalence rate was 17.6 Clonorchiasis cases combined with hepatocllular carcinoma and cholangiocarcinoma were 14 (M13, F1) and 4 (M3, F1). Prevalence rate (66.7%) in cholangiocarcinoma was much higher than that (14.6%) in hepatocellular carcinoma. The relative risk was 11.8. Average ages of total cases, hepatocellular carcinoma and cholangiocarcinoma cases were 51.9, 51.5 and 58.5 respectively. Ages of clonorchiasis cases combined with hepatocellular carcinoma were distributed from 30 throughout 60 and those combined with cholangiocarcinoma were limited in 50 and 60. From the epidemiological point of view, the evidence for a carcinogenic effect of Clonorchis infection is strongly suggestive.

Clinical Estimational of Gestational Age by Means of Neurologic Examination

Clinical estimation of gestational age at birth has been applied to 181 newborn infants who were born at Ewha Womans University Hospital from July to September 1979. Authors used neurologic examination based on evalution of passive tone, active tone and primary reflexes in order to estimate the gestational age, 1) Correlation coefficient between gestational age calculated from last menstrual period and clinical estimation of gestational age used in this study was 0.833 and regression formula was Y = 0.741+10.27. 2) There was no stastically significant difference in correlation coefficient between S.G.A, A.G.A., and I.G.A. groups. 3) Neurologic examination is objective and reproducible for estimation of gestational age in newborn infants.

Ectopic Pregnancy : A Clinical Evaluation of 435 Cases

This stdy is about cinical evalution and statistical analysis of 435 cases who were admitted and treated at department of obstetrics and gynecology, Ewha Womans University hospital from Jan. 1, 1971 to Dec. 31, 1979. The results were as follows; 1) The incidence of ectopic pregnancies was 1 in 38 deliveries. 2) The most frequent age group was in 25-34 years of age. The'youngest patient was 18 year old and the oldest patient was 49 year old. 3) igravida was 11.7%, ipara was 24.8% and 65.5% of total were experienced artificial abortion 4) The mean duration of infertility was 1 year 9 monthes and 3 cases were 9 years or more of inferility. 5) A history of previous laparotomy was 14.3%, repeat ectopic pregnancy was 7.8% and previous inflammatory disease was 8.7%. 6) Symptom was developed at 6.5 gestation week (mean). Symptoms were lower abdominal pain 94.7%, vaginal spotting in 84.7% , and shock state in 23.5% On pelvic examination, abdominal tenderness in 60.1, abdominal distension in 26.0%, and palpable adnexal mass in 39.2%. 7) Gravindex test was pregnant in 56% and culdocentesis was positive in 95.3% 8) Implantation site were tubal pregnancy in 91.7%, ovarian pregnancy 0.7%, cervical pregnancy 2.5% and cornual pregnancy 4.8%. 9) 69.5% of the cases were treated with salpingectomy and salpingoophorectomy was performed in 11%. 10) Of 435 cases, 1 death occurred who was dead at arrival.

Clinical Observation on the Urethral Syndrome Treated with Internal Urethrotomy

Urethral syndrome is becoming a recognized pathological entity and had been linked to various lower urinary tract symptoms such as irritative manifestations, enuresis and urinary tract infections. Despite the intensity of symptoms, these patients have not significant urinary tract infection that is demonstrable. A clinical observation was made on the 26 cases of the urethral syndrome treated with internal urethrotomy and the following results were obtained. 1. Age distribution revealed 18 cases (69%) in 41 and 60 year old and 7 cases (27%) under 40 year old. 2. Duration of symptoms were various from a few month to several years. 3. The most common complaints were 10 cases of urinary frequency, 5 cases of dysuria and 3 cases of urgency. 4. Cystoscopic findings revealed 8 cases of trigonal hyperemia, 7 cases of trabeculation and 6 cases of congestion and vesicles of the bladder wall. 5. 26 cases associated with urethral stricture were improved with internal urethro tomy.

Clinical Observation on Acne Vulgaris

The clinical studies were made on 500 cases of acne vulgaris who visited to Department of Dermatology, Ewha Womans University Hospital during from January, 1978 to June, 1980. The result were as followings; 1) Among 500 cases, 69(13.8%) were male patients, 431(66.2%) were female patients. 2) In the duration of the skin lesion from onset to be diagnosed, under 1 year was 63.0% (315 cases), and especially under 6 months was 50.4% (252 cases). 3) In the visiting frequency once was 38.4%(192 cases), and in the male was 42.0% (29 cases), and female was 37.8% (163 cases). 4) In the site of the skin lesion, face alone (90.0%), face and neck (3.8), face and chest (3.2%), face and back (2.0%), face and trunk (1.0%), were ordere. 5) In the factors affecting the severity of lesion, fatigue (26.8%), menstrual pentod (23.4%), gastrointestinal trouble (11.8%), cosmetics (9.0%), seasonal changes(5.4%), and unknown (23.6%) were revealed. 6) 309 cases having surgical treatment, 36.8% of once and 11.4% of twice were revealed.

A Case of Choriocarcinoma Following Term Pregnancy

Choriocarcinoma arises sometimes after hydatidiform mole, occasionally after abortion, and rarely after normal term pregnancy and ectopic pregnancy. Recently we had the opportunity to observe 26-year-old female patient who had choriocarcinoma following term pregnancy. In this paper, we report a case of choriscarcinoma following term pregnancy with a brief review of related literature.

Annular Pancreas in Infancy

One case of annular pancreas causing duodenal obstruction in 25-day-old girl was treated sucessfully by duodenojejunostomy at Ewha Womans University Hospital in July, 1980. She was one of the twins and premature baby in 37 weeks gestation. The hypothetical aspect of this anomaly, symptomatology, radiological findings, and the method of surgical procedures are discussed.

Pneumonia Simulation Myocardial Infarction in the Electrocardiography

We experienced the left lower lobe pneumonia simulating myocardial infarction in the eletrocardiogram. Eletrocardiographic changes were return to normal after the pneumonic infiltration had subsided. The finding in our case simply reconfirm the well-established principle that the surface electrocardiogram mirros the electrical phenomena of the heart, and it is important to draw attention to noncoronary syndrome that mimic ischemic heart disease.

2 Cases of Lymphangiography for the Diagnosis of Retroperitoneal Metastasis

Lymphangiography is the radiologic study that is not routinely performed because of its technical difficulty and highly skillful procedure. In cases of lymphoma and metastatic cancer of lymphatics, however, lymphangiography has a significant role in staging of tumor, determination of the therapeutic plan, follow-up study and evaluation of prognosis. The authors present 2 cases of successful lymphangiographies recently performed on seminoma and a penile carcinoma at Department of Radiology, Ewha Womans University Hospital

Leiomysarcoma of the Stomach

A case of leiomyosarcoma in stomach is reported with is rare disease and good prognosis rather than other adenocarcinoma of stomach. The tumor occurs in approximately equal frequency in both sex. It is seen at a somewhat young age than carcinoma of the stomach. This 26 year old female patient was admitted to our hospital on 14th Oct. 1930, with melena, dizziness and epigatstric mass. Physical examination revealed pale conjunctiva and questionable ill-defineed mass was palpated in the epigastric region. The C.B.C was obtained R.B.C.I. 8 million/mm3, HB. 5.0gm%, Hct. 15%, and W.B.C. 5,500/mm3 (Neutrophil seg. 70% and Lymphocyte 30%.) The upper G-I series showed multilobulated mass in lesser curvature of stomach. The massed have smooth and sharp bordered and overlying mucosa shows marked effacement. Superficial ulceration is also noted in the lesion. There is no evidence of rigidity or passage disturbance. Remain stomach and duodenum are within normal limit. Pathological examination was noted; multile, large, bulky intra-luminal massed are seen in the lesser curvature, with fungate into the gastric lumen and project subserosally. However, sections from tumor show non encapsulated multinodular tumor consisting of spindle and polygonal, moderately pleomorphic tumor cell by microscopy. There are some tumor cells arranged in whirl pattern with edematous highly vascularized stroma. Occasionally mitosis was vasible. This paper is concerned with our experience of a rare case of leiomyosarcoma of the stomach and reviewed with current concepts in the management in the literature.

A Case of Secondary Clear Cell Carcinoma in the Skull

The authors report a case of metastatic carcinoma in the skull presenting with pulsating mass of the right parietal region which was metastasized from clear cell carcinoma of the kidney.
